1. What Daybook is

My Medical Daybook (Daybook for short) is a personal record-keeping tool for a patient and the people looking after them, made and run by Mark 1 Apps Ltd, a company registered in England and Wales (company number 17510025). Contact: support@mymedicaldaybook.co.uk.

It is not a medical device, it gives no medical advice, and it does not replace your doctors, nurses or pharmacist. Everything it shows, including the summary lines, the flags, the food estimates and the AI explanations, is there to help you talk to your care team, not to decide anything for you. If you are worried about a symptom, contact your care team or NHS 111. In an emergency call 999.

2. Your account

One sign-in per person. Keep your password private; you are responsible for what is done with your sign-in. The household owner decides who is invited and with what access, and can remove them. By inviting someone you confirm that the patient agrees to them seeing the records.

3. Your records

They remain yours. You give Mark 1 Apps Ltd permission only to store them and show them to your household, as the privacy policy describes. You are responsible for what you enter and for checking it is right, especially medicine doses and times.

4. Using it fairly

Daybook is for personal, non-commercial use. Do not try to get into another household's data, probe or overload the service, or use it for anything unlawful. Do not enter anyone's information without their agreement.

5. Recordings

"Record the answer" and "Record the appointment" record people speaking. Ask them first, every time; the app asks you to confirm that you have. You are responsible for any recording you make and for what you do with it.

6. Daybook Assistant (the AI features)

They are optional. Daybook Assistant is an AI helper that runs on Claude, made by Anthropic. When you use it, the text or photo you chose goes to Anthropic in the United States, as the privacy policy describes. Anything it writes or reads, including explanations, summaries, suggested questions and details read from a label or plan, can be wrong. Check anything that matters with your care team.

7. Availability

Daybook is provided free and as it is. It depends on Google, Cloudflare, GitHub, your phone's maker and, for Daybook Assistant, Anthropic, and may be unavailable or change without notice. Mark 1 Apps Ltd may change, pause or withdraw it. If it is to be withdrawn you will be given reasonable notice and a way to take your records with you.

8. No warranty, limited liability

As far as the law allows, Mark 1 Apps Ltd is not liable for loss arising from your use of the app, from an error in what it shows, or from anything decided on the basis of it. Nothing in these terms limits liability for death or personal injury caused by negligence, or for anything else the law does not allow to be limited.

10. Ending
